Activity templates and one off activities

To speed up onboarding of new care recipients, we'll be adding activity templates where you can quickly select standard categorized activities to apply to the care plan.
This will also allow for reporting based on activity types.
With this project we'll also be introducing one off activities that happen once within a visit.
